Position Purpose: Under the direction of Population Health Management Organization leadership, this position is accountable for clinical aspects of assigned Population Health patient outreach programs. The goals of this program are to improve the health and well-being of the people and communities we serve through various patient engagement strategies such as health reminders, patient outreach, health promotion, member education, program enrollment, health coaching and other related services.
This position does provide patient care. Nature and Scope: This position is responsible for the coordination of the Population Health Patient Outreach Program. In collaboration with the Medical Director, referring physicians and interdisciplinary team(s), this individual assists with the identification and prioritization of Population Health/ACO quality measures, subsequent clinical outreach design, bulk clinical ordering, bulk member communication, delegation of work to non-licensed staff, and review of large amounts of diagnostic results. Other duties as requested are required. The incumbent must demonstrate the knowledge and skills necessary to coordinate care activities appropriate to the pediatric, adult and geriatric patient populations served and meets standards of quality as measured by the department age specific competency standards.

Coordinates patient outreach population management activities including: Identifying groups of patients overdue for applicable Health Maintenance topics within the EMR. Identifying, outlining and requesting operational patient reports within the EMR. Coordinating and arranging for follow-up services for populations of patients as appropriate. Identifying groups of patients not meeting clinical goals, such as BP control or glucose control, and coordinating processes around patient follow-up for services by protocol or as appropriate. Coordinating effective patient outreach campaigns with defined measures of success. Bulk ordering of lab tests, imaging tests, and procedures based upon pre-approved clinical protocols. Bulk patient communication of orders with emphasis on member engagement, education and action. Review of bulk test results and intervention per protocol. Working within an interdisciplinary team to coordinate workload skill-task alignment for various credentialed staff members. Coordinating pre-visit planning for specified patient populations. Working to meet the specific needs of members of the community. Clients of population health management programs include large and small employers, health plans, insurers, member groups, health systems, and government entities. Coordination of outreach programs, inclusive of: inbound telephone line services, and outbound care gap reminders, outbound new member activation, post-discharge follow-up, care management program enrollment, medication monitoring, chronic care management, and transitional care management. Serve as liaison between the clinical and administrative leadership teams. Leverage knowledge of health insurance and clinical care data to facilitate decision-making This position is expected to stay current with evidence-based guidelines with regard to preventive, chronic and acute recommended clinical guidelines.
Minimum Qualifications: Requirements Required and/or Preferred Education: Must have working-level knowledge of the English language, including reading, writing and speaking English. Bachelor s Degree in related field required. Master s degree highly preferred. Experience: Minimum 5 years nursing experience in the following areas is preferred: primary care, specialty care, preventive, case management, epidemiology and care management. Experience in public or community health preferred . Management and/or program development experience preferred. License(s): Ability to obtain and maintain a State of Nevada Registered Nurse license. Certification(s): none Computer / Typing: Must be proficient with Microsoft Office Suite, including Outlook, PowerPoint, Excel and Word and have the ability to use the computer to complete online learning requirements for job-specific competencies, access online forms and policies, complete online benefits enrollment, etc. EEO/M/F/Vet/Disabled
