Position Purpose: This position will be responsible for Acute Care Transfer Center, Admission Criteria, Discharge Planning and Acute Care Clinical Coordination, as well as discharge planning within Renown Skilled Nursing Facility and Inpatient Rehabilitation.
The Director shall be responsible for operating within an evidence-based best-practice Population Health Management Organization, including leadership development, understanding contracted relationships with necessary service providers and creating delivery partnerships with clinicians (both employed and independent) and community health organizations.
Nature and Scope: The Care Transitions Services division within the PHMO shall facilitate the following population health management objectives: (1) Coordination of ancillary and specialty service referrals, from acute care, skilled nursing facility and rehabilitation facility.; (2) Collaboration with clinicians to assure appropriate clinical coordination and discharge planning, (3) facilitation of seamless patient transitions of care across the health system, and (4) ensuring compliance with state, governmental regulations, JCAHO and (5) knowledge in the benefit structure of all plans contracted with the Renown Health System. This position does not make adverse clinical determinations, but relies on the physicians to ensure proper healthcare guidelines are met in relation to ambulatory and inpatient services.
Experience and/or responsibilities include, but are not limited to the following: developing and implementing operating plans as well as analyzing financial, clinical and quality data; ability to lead a comprehensive assessment of regional community health networks to create the necessary member touch points; delivering a scalable and sustainable care model to ensure Triple Aim performance improvement strategies align with all potential population health management models.
Direct Oversight of the following: * Transfer Center o Initial Clinical Review of Observation patients for clinical appropriateness o Clinical Review of incoming and outgoing transfers for clinical appropriateness * RN Case Managers and Social Workers in Emergency Room * Acute Inpatient discharge planning o Screen and Identify DC needs o Part of interdisciplinary care team identifying needs upon discharge including social determinants, functional level and need for transitional care setting
This leader shall also maintain accountability for financial performance, human resource management, planning, business and/or clinical operations, project management, and facilitating discussions regarding the necessary IT infrastructure to support operations.

Minimum Qualifications: Requirements Required and/or Preferred Education: Must have working-level knowledge of the English language, including reading, writing and speaking English. Experience: Minimum of five (5) years of management experience, with at least two years of Acute Care Experience required. License(s): RN or MSW clinical licensure required. Certification(s): Professional and/or clinical certifications preferred. Computer / Typing: Must be proficient with Microsoft Office Suite, including Outlook, PowerPoint, Excel and Word and have the ability to use the computer to complete online learning requirements for job-specific competencies, access online forms and policies, complete online benefits enrollment, etc.
